Insurance
We take most major insurances. Additionally, we have a number of programs to provide care to the LGBTQ+ community including Gender Affirming Care, PEP, PrEP, and HIV. As a last resort we have low-cost options for care and will address any payment concerns at your initial appointment. Please contact your insurance company if you need to see that you are covered. New Patient Registration
All new patients should complete the New Patient Registration Form. The clinic will then contact you to discuss clinic availability and needs
Existing Patients
Existing patients should book appointments through the patient portal or call your desired location for an appointment.
